Our team is made up of skilled professionals who work together to provide holistic care tailored to your needs.
Registered Nurse (RN) Case Manager
Our RN Case Managers play a vital role in coordinating your care, ensuring that you maintain the highest quality of life. They work closely with your Attending Physician to manage pain and symptoms related to your diagnosis.
- Perform comprehensive patient assessments during each visit.
- Educate patients, caregivers, and families on medication use, medical equipment, symptoms, treatment, and the disease process.
- Available 24/7 for consultation or emergency visits, ensuring support whenever it's needed.
To reach a nurse during normal business hours (Mon-Fri. 8:30am – 5:00pm) or after hours and on weekends please call our main office number 469-630-2538.
Certified Nursing Assistant (CNA)
Our CNAs, also known as Hospice Aides, provide essential personal care and assistance with daily living activities, tailored to each family's needs.
- Assist with bathing, grooming, mobility, and other personal care tasks.
- Offer support with light house duties directly related to patient care, such as laundering clothes, straightening the patient’s room, and warming meals.
- Visits are scheduled based on the patient's care plan, ranging from once a week to daily.
- CNAs are not responsible for family meal preparation or family laundry, etc.
- CNAs are not responsible for administering any medications
Licensed Mastered Social Worker
Our Social Worker is here to support you and your family through emotional and practical challenges, including:
- Providing guidance on community resources, legal, and end-of-life planning.
- Offering support for financial concerns, stress management, and unresolved feelings related to the illness.
- Available for phone consultations or personal visits to help navigate any difficulties that arise.
Patients, caregivers, and family members are encouraged to reach out for assistance with any unmet needs.
Chaplain and Bereavement Coordinator
Our Chaplain provides compassionate spiritual care tailored to your beliefs and needs.
- Offers emotional and grief support to patients, caregivers, and families.
- Works with your existing clergy or provides direct spiritual guidance, respecting all faiths and beliefs.
- Assists with funeral planning and can perform services if requested.
The Chaplain is available by phone or personal visit to provide comfort and guidance through challenging times.
Volunteers
Hospice Volunteers are an integral part of our care team, offering companionship and practical assistance.
- Help with tasks like errands, light housekeeping, and emotional support.
- Sit with patients to give caregivers a needed break.
- Available to visit at times that best suit the patient’s and family’s needs.
Volunteers are trained by our staff to ensure they can meet the special needs of hospice patients and their families. If you need a Volunteer, please contact our office in advance to schedule.
