All clinical staff services, including our physician Medical Director, RN Case Manager, Home Health Aide, Medical Social Worker and Chaplain. Volunteer services are also available

The Care You Need, Wherever You Need It

Our care is provided within the familiar setting of wherever the patient lives and calls home:

Their own home or apartment
A friend or relative’s home
Assisted living facility Nursing home
Independent care facility
Hospital

However, the patient does not have to be home-bound.

Who’s Eligible for Hospice Care?

Eligibility for hospice care typically includes individuals with a terminal illness and a life expectancy of six months or less, as certified by a physician. Our team can guide you through this decision-making process with empathy and expertise.

In order to be eligible to elect hospice care under Medicare, an individual must be entitled to Part A of Medicare and certified as being terminally ill by a physician and having a prognosis of six-months or less if the disease runs its normal course.

Honoring Those Who Served

At AlēvCare Hospice, we proudly partner with the US Department of Veteran Affairs through the 'We Honor Veterans' program. This initiative ensures that terminally ill veterans receive the compassionate care they deserve, tailored to their unique needs.

Veterans often have specific needs at the end of life due to their unique and sometimes traumatic experiences. We provide emotional and spiritual support that can be especially meaningful to those who served our country so selflessly.
